Introduction

Upon receiving the Royal commend from His Majesty the King to prepare the plan for the worst case scenario of the worst CVOID-19 situation. The Ministry of Health and Khesar Gyalpo University of Medical Science of Bhutan (KGUMSB) has planned to train additional 300 Desuup as frontline workers (Desuup Plus) to support the existing health workers. This group will be responsible for managing asymptomatic and symptomatic cases anytime they are called for duty with the ratio of 1:10 of desuup plus-patient. A 6-day training curriculum was reviewed and revised to 7 day by KGUMSB as per projection by the Technical Advisory Group and identified needbased skills to manage the cases. The Desuups will be trained in basic healthcare management skills in eight locations namely Thimphu, Paro, Punakha, Wangdu, Tsirang, Bumthang, Mongar, and Trashigang. All the documents required will be submitted to BMHC as a continued training of Basic Frontline responders with additional skills requirement on administration of oxygen and dead body management. 

General Objectives 

The training is designed to build the competencies of Desuup on the basic health care and management of asymptomatic and symptomatic COVID patients in the facility isolation and management of the dead body. 

Specific objectives

By the end of the training, the participants should be able to;
1. Understand the basic of COVID-19 and new variant
2. Understand the roles and responsibilities
3. Understand the importance of adhering to code of ethics

4. Perform the following procedures independently;
a. Admission and discharge
     b. Donning and doffing
     c. Infection control
     d. Vital signs
     e. Administration of oxygen
     f. Preparation of dead body
     g. Body mechanic
     h. Documentation: recording and reporting

Selection and eligibility criteria

The selection of participants for the Desuup plus training will be done by the office of De-sung as
per the projection plan. Stringent criteria were set and only upon fulfillment of the criteria, 300
Desuups will be selected. Desuup plus will be working at the isolation facilities where
symptomatic COVID-19 patients requiring Oxygen will be kept.

Following are the eligibility criteria:

1. Above 20 years and below 40 years on the day of registration
2. Free of chronic disease such as heart disease, diabetes, hypertension, kidney disease, lung disease including history of tuberculosis, mental disorder and any other disease that could
be aggravated by stress.
3. Willingness to work in the high-risk zone (facility-based isolation)
4. Un-employeed
5. Minimum class 12 pass

Training period and number of participants 

The selected Desuup will undergo 7 days training as frontline responders (Desuup Plus) for COVID-19. The training will be provided by the team of faculty members from KGUMSB in collaboration with the Ministry of Health. The training will be conducted in 10th batches at eight different locations as per the schedule mentioned in the Annexure 1. Maximum of 30 participants in a batch.

Trainers

The trainers are mainly professors and lecturers of KGUMSB.

Logistic arrangement

The Ministry of Health will arrange the logistics of the training. The contact person of focal
officials of the Ministry of Health, KGUMSB and De-suung are mentioned in Annexure II.

Brief training Content

The training will focus on the following competencies to manage symptomatic patients requiring
oxygen and management of the dead body.
Knowledge

1. Basic of COVID-19
2. Chain of infection transmission
3. Normal vital signs
4. Code of ethics
5. Principle of body mechanics
6. Administration of oxygen
7. Management of dead body

Skills

1. Carry out admission and discharge
2. Follow strictly severe steps of washing hands and using hand sanitizer
3. Donning and doffing of PPE
4. Measure blood pressure, temperature, pule, respiration and oxygen saturation
5. record the findings correctly
6. prepare bleaching solution
7. clean, decontaminate and disinfect linen, equipment, articles, facility and waste
8. Management of waste of the facility
9. Maintain body mechanic
10. Administration of oxygen
11. Management of dead body

Attitude

Feel a sense of service, purpose and commitment

Responsibilities

The primary responsibilities of these volunteers are depending on the needs to assist nurses
stationed in the isolation facilities but not limited to the following points;

1. Support nursing staff in overall management and supervision
2. Monitoring worsening of signs and symptoms
3. Perform all the skills acquired in training
4. Report the finding to the nurse and any deviation on the findings
5. Distribution of meals
6. Facilitate refill for any chronic conditions
7. Adherence to PPC protocol as prescribed
8. Management of waste in the facility
9. Ensure prevention of infection as per guideline
10. Facilitate counseling services when required
11. Ensure all the occupants follow the DO’s and DON’Ts of isolation facilities.
12. Manage the dead body as per guideline

Deployment Plan (WHEN, HOW, WHERE)

The desuup plus are expected to function as nurse assistants in identified isolation facilities in the country. The office of de-suung will deploy as per need activation from the MoH.
